NSDTA/APHSA Awards Application  

To Nominate Someone:

  1. Complete the application form template.

  2. Include as part of the application:

  • A nomination letter

  • An abstract that does not exceed two typewritten pages

  • Any accompanying material that supports the nomination (no more than 5 pages)

    • Documentation of quality (for example, level III or IV training evaluations, cost savings documentation, or outcomes achieved). (No more than 5 pages)
    • DVD, CD or VHS tape will be accepted if they are used in substitution for the documentation or supporting materials. 
    • The committee does not accept complete copies of curriculum or programs as part of the supporting materials.
  1. Email the above nomination form, letter, abstract, and other materials to:

Deadline for nominations is May 15, 2006
Award Winners will be honored at the NSDTA Annual Professional Development Institute on September 10 - 13, 2006 in Minneapolis, MN.  Winners will receive one free institute registration.
